Oregon Independent Living Survey

Hi! I hope you are doing well, that you enjoyed the great weather last week, and you're excited for a great summer in Oregon. If you are over the age of 14 and you haven't taken the survey yet, I would like to ask you to help the Independent Living Program to determine what services best meet your needs and will help you be better prepared for your transition to adult living.

Details on Oregon's New Chafee Medical Program

Effective May 1, 2010, DHS is implementing HB 3664, the Chafee Medical Program for youth who age out of the foster care system (left a substitute care placement at age 18 or older). Unfortunately, if a youth aged out of care prior to May 1st, they are not eligible for this service.
